The only delivery method for the Fall Survey will be through email. Please email the survey to schoolsurvey@archchicago.org by October 1st.  Please include your school name and city in the subject header.

Please Note: All questions on the survey must be completed.  If a section or question does not apply to your school, please type "DNA".  If sections are incomplete or insufficient information is provided, we will call you to clarify.
